MEDIA CONSENT DETAILS

By selecting “Yes, I consent to be in photos, videos, and audio clips taken during BASE Camp”, you are agreeing to the following:

1. You give your consent and authorize BASE Camp personnel and NewYork-Presbyterian Hospital (“NYPH”) staff to photograph, videotape, record and/or to have information about you disclosed in connection with production of the BASE Camp, which will be produced and distributed by BASE Camp.

2. You understand and agree that such photographs, video recordings, and/or audio recordings may be used for scientific, educational, and/or public relations purposes – such as on our website, brochures, flyers, newsletters, presentations, and/or periodicals.

3. You understand that BASE Camp may include your name, likeness, picture, image, voice, personality and/or personal identification information (collectively, your “Information”). 

4. You grant BASE Camp and NYPH and its subsidiaries, affiliates, licensees, employees, agents, academic affiliates (i.e., Columbia University and/or Cornell University), and any persons or entities authorized by the above (collectively, “NYPH”) and BASE Camp the right to use and disclose your Information in connection with the production, distribution and promotion of BASE Camp (or excerpts of BASE Camp or derivative works made from BASE Camp) in all media and distribution channels of any kind, whether now known or hereafter devised, worldwide, in perpetuity. You understand that BASE Camp intends to use and disclose your Information in connection with BASE Camp and to make derivative works therefrom as they, in their sole discretion, shall deem appropriate.

5. You release and discharge NYPH and BASE Camp from any and all claims, demands or causes of action that you may now have or may hereafter have for libel, defamation, invasion of privacy or right of publicity, infringement of copyright or violation of any other right of yours arising out of the exercise of the rights granted herein.

6. You understand that you will receive no financial compensation from NYPH for participation in BASE Camp, you have no rights of ownership to BASE Camp or rights to approve BASE Camp, and NYPH and BASE Camp are not obliged to include your Information in BASE Camp.

7. You understand that this media consent contains the entire agreement and understanding between NYPH and you regarding the subject matter hereof and no oral understandings have been made with regard thereto.

8. This media consent, and any claim, controversy or dispute arising under or related to this media consent, shall be governed by and construed in accordance with the laws of the State of New York, without regard to principles of conflicts of law. 

9. You have been advised and understand that:

You have the right to request that such photographing or recording be stopped; and, You have the right to rescind your consent for use up until a reasonable time before the recording or photo is used. You have the right to refuse consent to photographing or recording and understand that this decision will not affect your right to participate in BASE Camp programming.
